雇主需为雇员登记接种疫苗

 

砂拉越灾难管理委员会于27.03.2021宣布，由于近来不断爆发职场感染群，砂雇主需为雇员登记接种冠病疫苗。各领域的雇主，包括服务业、酒店业、建筑业、制造业和农业领域的雇主，必须在今年4月12日之前，向各省公署提交雇员名单，以作为接种冠病疫苗的登记用途。

1. 在线填写表格 Google form at Residents Office Sibu website (https://sibu.sarawak.gov.my)
2. 下载表格填具后电邮至该署 Soft copy excel (download at https://sibu.sarawak.gov.my) submit / email to admeventinfo@gmail.com

1. 填具表格，植入公司信笺 Fill in the form (as attached) insert your company letterhead
2. 书面函件 Issue a letter with the title “Registration of Employees for the Covid-19 Vaccination Programme”
3. 提交至古晋县公署 Submit to District Office (due to closure of Residents Office till 11.04.2021)
